Please join us for Gifford's annual Advanced Care Planning Fair where you can complete or update an advance directive and have questions answered around advance directives and long-term care planning. Gifford staff will be on hand to answer questions and assist with paperwork. A number of community partners and service providers will also be present for you to interact with. Community partners include: Support and Services at Home (SASH), Council on Aging, Home & Able, Visiting Nurse and Hospice (VNH), TriValley Transit, Gifford Health Connections, Gifford Chaplaincy Program, Bayada, Capstone, and Day Funeral Home.
